Enrollment Requirements

To enroll in the program, please complete and submit the application below. You will be prompted to submit a valid photo ID plus 2 of the following:

          • Valid union card
          • Current agency ZED or comp card
          • Business card
          • Cosmetology or esthetician license

 

For students, please submit a valid Photo ID and a proof of current enrollment.
